Annual Individual Membership
Child $279 (Age 12 and younger)
Membership fee is due in full upon joining and expires one year later on anniversary date.
Payments for services are due when services are rendered. Visa, Master Card, and Discover credit cards are welcomed.
Discounts apply to all services.
Your membership is NOT a dental insurance plan and can ONLY be used at Genovese Dental. The plan is non-transferrable and cannot be combined with any other dental insurance coverage. The plan cannot be used in conjunction with any other financial convenience programs, such as Care Credit or Senior Discounts.
Discounts under your membership do not apply to any treatment completed prior to or initiated prior to membership.
Dental Care Membership includes: (per 12 month period)
– Two Dental Cleanings or Two Periodontal Maintenance Cleanings
– Two Complete Oral Examinations including Cancer Screenings and Annual Periodontal (gum) Measurements
– Cavity Detecting X-rays, Intraoral Camera Examinations, Two Fluoride Treatments for Children (12 years of age or younger.)
– 20% discount on ALL other Dental Treatment. Fees are based upon the usual and customary charges.
– No Yearly Maximums!
– No Deductibles!
– No Pre-Existing Conditions/Exclusions!
The plan puts YOU in control of your Dental Treatment
It is the sole responsibility of the member to maximize their benefits by arranging the appropriate appointments within the 12-month period of membership. If the appointments are not used, the member will not be entitled to a refund or extension of benefits.
